Police Department Records Request Form & Fee Schedule
We have improved our Open Records Form so that you will be able to type right on the form, save it on your computer and e-mail it to the Police Department. (campbellpd@townofcampbellwi.gov) If you do not have a computer or access to the internet, you can pick up an open records request at the Town Hall or the Police Department. You will still need to pick up and pay for your request at the Campbell Town Hall Main Office 2219 Bainbridge Street La Crosse WI 54603 during normal business hours.

New Parking Contestment Form
Citation Payments (agency plc code 5981). If the citation you received is for the coulee region joint municipal court, you can pay it directly to the CRJMC in Onalaska. (court plc code 2305)

Citizen Report Form
The Campbell Police Department is introducing a new citizen reporting form that will allow residents to report minor theft crimes, minor damage to property complaints, lost property, suspicious activity or persons and nuisance complaints online. One word of caution, filing a false police report is a crime and ALL CASES OF FALSE REPORTING WILL BE PROSECUTED!
Instructions for completing the citizen report are located in the TABS Section above, right next to the citizen report form itself. You may click on the instructions and print them out for completing your report. Once your report is complete, electronically sign it and email it to the police department. Once it is received, an email will be sent back to you with an assigned Case Number for your report.
All fields on the report must be completed to the best of your ability and all must include your full name, address, phone number and birth date to be assigned a case number. If you are reporting a crime for someone who is unable to do so, list that persons full name, address, phone number and birth date in the report in addition to your own information.
Remember this form is ONLY for reporting:
- Thefts under $250.00 with no suspects
- Damage to Property under $2500.00 with no suspects
- Lost Property
- Suspicious Activity or Persons you personally observe
- Nuisance Complaints you personally observe
All other incidents you want to report to the police should be reported to an on duty police officer. The citizen report form is only for incidents that occur in the TOWN OF CAMPBELL. Incidents occurring outside the Town of Campbell should be reported to the jurisdiction where they actually occurred.
